Job description: Responsible for assisting the Care Coordinator for the Chronic Care Management (CCM) Program and Program Director for Patient Centered Medical Home (PCMH) accreditation. Duties include: identification of patients eligible for CCM through the PCP offices, hospital discharge planners, and referring organizations; coordinate patient referrals for those enrolled in CCM program (disease-specific specialties including mental/behavioral health referrals), work with JCH Health Care and referring organizations to help enhance the care the patients receive, assist with PCMH accreditation and quality metric reporting. Through day-to-day responsibilities, LCSW may recommend process flow changes in order to continue to help enhance the patient care and/or PCMH accreditation needs. LCSW may provide counseling sessions if determined a need during the CCM program. This position is 100% dedicated to the HRSA Small Health Care Provider Quality Improvement Program Grant and is grant-funded.

Description (Substance Use and Opioid Use Disorder (SUD/OUD) counselor)

Work collaboratively with prescribers to provide care management services, including counseling services to patients enrolled in the Medication Assisted Treatment (MAT) program. This position works closely with the JCH Medical Group health care providers that provide addiction services. This is a 100% HRSA Grant Funded Position through the HRSA RCORP – MAT Expansion Grant, #GA3RH33623.

1. Identify current opioid prevention/recovery efforts within the counties
2. Assist MAT patients with completing insurance applications and enrollments
3. Connect patients and providers with local resources for Substance Use (SUD) & Opioid Use Disorder (OUD) services
4. Provide counseling to SUD/OUD patients enrolled in the MAT program
5. Document patient interaction and events consistent with industry standards
6. Create appropriate treatment plans for each patient and review with patient and provider
7. Ensure patients have access to the appropriate resources for successful treatment and recovery

Requirements

• Preferred: Current IL Clinical Social Worker license.
• Minimum: MSW and able to become LCSW within 2 years
• Comply with HIPAA regulations.
• Represents JCH in a positive/professional manner.
• Comply with corporate compliance policies regarding ethical/legal business practices.
• Maintain knowledge and remain in compliance of The Joint Commission and rural health regulations.
• Knowledge of correct coding principles

Please note: Jersey Community Hospital is hiring three LCSWs. We are in need of a mental health counselor, a Substance Use and Opioid Use Disorder (SUD/OUD) counselor, and an adolescent counselor.
