Job title: Social Worker Dementia Care Specialist Company: Trempealeau County Job description : Social Worker – Dementia Care Specialist PURPOSE OF POSITION: The purpose of this position is to provide Trempealeau, Buffalo and Pepin Counties with the knowledge regarding older adults, dementia, dementia care and support, family and informal caregiving needs as well as challenging behaviors. This position is unique in the aspect that it will serve all three counties although Trempealeau County will be the primary office and employer. The Dementia Care Specialist Social Worker will also provide community development, marketing and public outreach for the ADRC Services. This position would work with community partners to help connect the community to the ADRC Services and local volunteer services.
