Responsibilities include regular liaison with members of the treatment team, discharge planning, aftercare follow-up and liaison with community agencies as needed. The Social Services Clinician conducts the psychosocial assessment for assigned patients. The Social Services Clinician provides family safety planning meetings if appropriate. The Social Services Clinician helps to coordinate and to provide family services. The Social Services Clinician facilitates or co-facilitates group therapy.
