Coordinates a variety of services including crisis intervention, bereavement, client advocacy, referrals, prevention, education, discharge planning and implementation, and psychosocial assessments. Collaborates in the development and implementation of performance improvement initiatives. ESSENTIAL FUNCTIONS OF THE ROLE Conducts and documents detailed psycho-social assessments to interpret social, environmental, and medical needs of the patient.
