Job title: Medical Social Worker (LCSW) Company: Social Workers Job description : In collaboration with patients, families (as defined by the patient), providers and staff across all disciplines, the person in this position is responsible for providing mental health and substance abuse interventions, completing psychosocial assessments, and addressing the social determinants of health. The person in this position serves as an integral part of the multidisciplinary healthcare team and serves as the patients’ advocate in addressing their social work and behavioral health needs. This grant funded position will serve as a fundamental team member to continue to cultivate CommUnityCare’s team-based care model initiatives.
