DESCRIPTION OF WORK

This is the entry level, trainee care manager position. Upon successful completion of the 6-month probationary period, employee will be promoted to an Aging Care Manager 2 with salary increase. Aging Care Managers are trained to assess the needs of older adults; develop and implement care plans; provide advocacy and ongoing care management; and protect consumers from abuse, neglect, exploitation and abandonment. Our goal is to improve the quality of life for older adults by providing them with, and access to, a range of community and home-based services. We are dedicated to empowering and advocating for the rights of older adults while supporting their family and other caregivers. This position may, at times, require flexible work location/environment.

• Full-time employment.
• Work hours: 8:30 am – 4:30 pm; Monday – Friday with a 30 minute lunch period.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

Qualifications:

• Minimum Experience and Training Requirements – A Bachelor’s Degree which includes or is supplemented by 12 college-level credit hours in sociology, social welfare, psychology, gerontology, or other related social sciences; OR three and one-half years of experience as an Aging Case Aide and 12 college-level credit hours in sociology, social welfare, psychology, gerontology, or other related social sciences; OR any equivalent combination of experience and training which includes 12 college-level credit hours in sociology, social welfare, psychology, gerontology, or other related social sciences.

Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including, but not limited to, PA State Police, Child Abuse history clearance, and FBI clearance.
• A conditional offer of employment will require satisfactory completion of drug screening test.
