The Eastern Colorado Veterans Health System (ECHCS), Social Work Service is currently recruiting for one (1) Senior Social Worker for Home Based Primary Care (HBPC) in Pueblo, Colorado. This position requires the incumbent to operate a government vehicle and must be licensed to operate a motor vehicle. Submission of a valid driver’s license must be included with application. The Senior Social Worker is directly responsible to the Geriatric and Extended Care Program Manager within the Care Management and Social Work Service under the office of Associate Director of Patient Care Services. Practices independently, exercising initiative and independent judgment in day-to day activities. Supervision/consultation is of an administrative and professional nature. The incumbent facilitates efficient and appropriate delivery of clinical and case management services across episodes of outpatient care within the Veterans Affairs Medical Center (VAMC). The incumbent plans, develops, implements and evaluates the care provided through Home Based Primary Care, a specialty primary care clinic. Work involves oversight of a complex array of services, both clinical and administrative. Work requires the exercise of mature professional judgment and flexible use of administrative, leadership, and social work skills. The population served has unique health needs, mental health issues, and the necessary involvement of family and other agencies often result in serious and complicated cases. Patients are unable to access traditional primary care so are provided this level of care within their homes. Patients present with a wide range of psychosocial and environmental problems that require specialized knowledge in chronic and terminal illness, palliative care, and end of life care. The Senior Social Worker (HBPC), Facilitates efficient and appropriate delivery of clinical and case management services across episodes of outpatient care within the Veterans Affairs Medical Center (VAMC). Will often be the only clinician visiting the residence and must independently manage any presenting crisis to include assessment and placement of psychiatric holds, notification to medical team members and/or emergency services, and other clinically appropriate interventions. Plans, develops, implements and evaluates the care provided through Home Based Primary Care, a specialty primary care clinic. Will identify areas for improvement, gather relevant data, assess the data, develop and implement ideas for improvement and evaluate efficacy of improvements. Will identify high risk patients and proactively provide case management services. Is the designated expert concerning the provision of clinical and case management services for Veterans within the Home Based Primary Care team. Coordinates psychosocial services provided to the Veteran. Coordinates and collaborates with the interdisciplinary teams establishes collaborative relationships with physicians, psychologists, pharmacists, and other disciplines to foster their involvement in the case management of Veterans. Provides education and training to staff from other disciplines, students, and volunteers as assigned. Serves as a mentor to other social workers, particularly those at the entry level. Meets the needs of customers while supporting the VHA, VISN, facility and service missions and business plans. Respects diversity and consistently communicates and treats customers (patients, visitors, volunteers, students, facility staff, community representatives) in a courteous, tactful, respectful and fair manner.
