Job description: Responsibilities

Working in collaboration with other members of the patient’s care management team, the Innovations Telehealth Clinician utilizes effective communication and customer service skills to provide essential care coordination including compassionate presence and support, identifies the need for early intervention for symptom management or psychosocial needs, provides ongoing reinforcement of patient’s case management plan, performs teaching related to care of patient, medication management, and supports goals of care and advanced care planning needs as well as assist in identification of advanced illness end of life signs in accordance with his/her professional skills. The Innovations Telehealth Clinician demonstrates technical competency in the use of computer telephony systems and computer applications, maintaining accurate patient medical records and scheduling virtual phone appointments as well as assessing and supporting the delivery of services consistent with social determinants of health as needed.

Qualifications

Experience Requirements:

Must have five years of clinical practice experience. Experience in hospice, home health, community health, private duty, and/or outpatient services is preferred. A minimum of one-year experience with telehealth systems and service programs is preferred.

Education Requirements:

Must have a degree in Social Work or Nursing from an accredited program. A BSN or LCSW is preferred.

Required Certificates and/or Licenses:

Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice; or licensure from compact state. A certification in Case Management is strongly desired, either at hire, within 12 months of employment, or within the requirements as outlined by the CCM certification standards.

Drivers’ License:

Must have driver’s license in good standing and access to a vehicle during working hours.
