Job description: The Social Services Director in a nursing home directs, establishes, and plans the overall policies and goals for the social services department.

Duties of Director of Social Services:

  • Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psycho-social needs.
  • Help Oversee/Manage the Social Service Teams daily tasks/job duties.
  • Identifying individual social and emotional needs
  • Developing and Maintaining individualized care plans
  • Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
  • Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
  • Build connections with area healthcare partners to help with admissions and census growth
  • Coordinate the pre-admission process and acts as liaison between the customer and the Facility.
  • Facilitate an admissions process that is in accordance with State and Federal Regulations

Requirements of Director of Social Services:

  • Bachelor’s Degree in Human Services; Psychology or Social Work -Long Term Care / Sub acute Experience
  • Required. Social Work License
  • 1-3 years of managerial experience
