Job description: iCMP Social Work position (40 hours)

We are seeking a social worker for iCMP, a care coordination position for patients with primary care at MGH’s Internal Medicine Associates on the main campus. Excellent clinical, collaborative and consultation skills, required to work closely as part of a multidisciplinary team. Some of the coordination and patient contact done telephonically or through telehealth.

Qualifications
