Job description: Overview

The Coordinator for Population Health and High Value Care provides support to the various Optimal Care and Population Health Programs through developing, coordinating and maintaining a variety of informational tools and sites for Optimal Care. This person acts as a resource for teams and leaders within the various programs to facilitate ongoing improvement of communications channels within the programs, across the organization, and external to the organization as indicated.

Responsibilities/Job Description

  • Provides coordination of information across various digital platforms including, but not limited to Modern Workplace tools such as Teams, and SharePoint as well as more traditional tools such as Power Point and Excel. Will need to develop standard communication tools and templates that support organizational engagement in Optimal Care programs and improve organizational performance. Also may coordinate communications intended for external audiences such as Payers, Key Stakeholders, and Patient Populations.
  • Develops appropriate proficiency with multiple organizational tools such as Policy Tech, Epic EHR and others as needed to facilitate programmatic communications successfully.
  • Navigates organizational structures and communications pathways across a complex organization spanning independent and employed practices, as well as academic practices. Successfully navigates and communicates across various Service Line structures.
  • Collaborates successfully with key stakeholders and contributors in order to deliver information in a timely and efficient manner.
  • Communicates complex content with clarity through written, verbal, and visual content. Is able to visually structure tools, presentations, and web content to facilitate clarity and ease of use for consumers of Optimal Care information.

Qualifications

Required

Education

  • Associates degree or vocational training or equivalent education/experience.

Experience

  • 1-2 years’ experience with digital platforms such as Teams, SharePoint, and other Microsoft programs such as Excel and PowerPoint along with creating presentations.
  • Demonstrated excellent communication skills, both verbal and written.
  • Attention to detail.
  • Good organizational and collaboration skills.
